Objectives

The formation of Concept: CARE was premised on the identification of a need
for a community based home health care operation which would set a precedent of
quality care standards for future generations and serve as a model for the health care
industry. It was recognized that the overall quality of life for many of our
Westchester elderly and disabled needed health care interventions and decision
making capabilities done expediently by a local provider of services, people who
themselves lived in their community. As a company, Concept: CARE would
combine community resources, health care networks and advocacy for the population
it serviced and utilize the alliances previously established. Concept: CARE identifies
itself as a health care family committed to health promotion, supportive intervention
and advocacy in conjunction with and/or absence of a family nucleus.

To provide comprehensive health care services through a framework service
     organization for the planning, directing, coordinating, provision and
     improvement of health care and services that are responsive to the community
     and client needs so as to improve client outcomes.

To provide effective and cost efficient health care services in the security and
     comfort of the client's own home.

To facilitate optimal rehabilitation potential for each client based on the
     established plan of care.

To promote understanding of good health practices; to provide physical,
     emotional and psycho-social care to terminally ill clients and their families; to
     prevent unnecessary institutionalization; and to facilitate early discharge by
     providing technically feasible treatment in the home.

To provide counseling and guidance in locating and utilizing personal and
     community resources to assist with situations that may contribute to poor health.

To foster an environment conducive to learning by providing ongoing
     orientation and participation in in-service and continuing education programs
     for all associates.

To employ the highest standards of ethics as they relate to the client, associate,
     community and all business practices and clinical outcomes.
